Submit form not working, permalink issue?

Using custom page templates.
1st page has a form and submit button.
When you hit the submit button, it shows the correct url in the browser address bar, but gives a page not found. If you just go to address bar and hit enter, the URL works.

So it has something to do with the permalinks I think. Cause if you embed the same url in just a hyper link, the link works, but when using it in a form submit post, it does not.

Any ideas on this?

Here is the test site that shows the issue:

http://mustangwebsites.com

Then click on page Test Form.

Then Enter your name, and submit, you get page not found.

If you dont enter any values in the name field, it finds the form post page.
