    Default need a suggestion for a form handler plugin.

    I have a website where I need to create a bit of a questionnaire/order form and submit it to an email address.

    That is the basic requirement.

    It's a bit more than a simple contact form but not exactly something very sophisticated. I need to collect name, email, phone, address and then have them choose from products and about 10 set questions before submitting to an email. I'd like to format the form a bit into a few columns and maybe some headings to improve the ease of reading and use.

    I've found cformsII to do this but not sure if it's the right fit as customizing the form's design seems tough.

    Any other suggestions?

    I'll throw my own plugin into the ring: Visual Form Builder. It has everything you are asking about except perhaps the columns. But, if you are customizing the design you can simply change the CSS to allow for that.

    If you don't like mine, you can check out Gravity Forms or Contact Form 7.

    Sounds like Gravity Forms would work well for this. You could also use something like PollDaddy to create a survey and after the initial form or product selection is performed, that user is redirected to PollDaddy to answer the set questions.
