Looking for a plugin that allows me to assign images to a custom field

I cannot seem to find a simple plugin which allows me to upload or choose images from the media library for custom fields. I want to be able to define some custom fields (per post/page) which would contain some images to display for that post or page so that a client can easily change the images themselves without having to know the image URLs. Anyone know of a plugin like this?

Thanks for the suggestion, just tried that plugin out and it doesn't seem to handle images that well, it doesn't use the media library at all.

Try http://r2h.me/cft - It doesn't use the media uploader to upload the images, but it does store the images in the media library and you can configure it either to relate the upload image to the post or to not relate them
